Export multiple form to PDF?

Hi,

I am looking for how to export multiple contents on one or more pages.
Not from a grid, but from a form.

Example :

HEADER : NOTES From : First name | Last Name | Etc.
CONTENT :

(Note 1) Date, duration
Description (multiple line)

(Note 2) Date, duration
Description (multiple line)

Do you have any idea how to do this?
