Images not showing in PDF reports

Hi All,
I am having a difficulty creating a PDF report with some images from a database. Images are stored in the database as name and actual files stored in a seperate sub folder.

When created PDF report it only shows the name of the file but does not display the actual image. Please refer to attached image. What could be the reason to not to show these images?
