Missing Prompts

I have a problem with Form prompts have been missing. Tired everything I could think of but nothing helps.
Here is an example of settings for one of the fields. It all looks fine

[ATTACH=CONFIG]n66431[/ATTACH]

When I run App the form shows no prompts in any field

[ATTACH=CONFIG]n66432[/ATTACH]

Any ideas or tips ?

Check the block settings. At the block level you can disable (i.e. not show) all fields labels.
