Multi-upload "The table or the multi-upload fields were not informed."

I have followed instruction … watched a couple of different videos … and can’t make this work. The issue I get when attempting to run the form is “The table or the multi-upload fields were not informed.” … the files-table was created verbatim from code provided in the tutorial. What am I doing wrong? I have the fields configured like this:

This is the tutorial I am following. Note that it shows only 2 fields with values - but the db table has 5 fields.

Any idea what might be causing this issue? The error occurs when I attempt to run the form.

Started over with new form. Made sure that the form would display before adding the upload field.

Good morning,

Can you show more of how your form is setup?

I have this setup and working on a few forms.

The datatype I have is Document (database)

These are how i have my fields setup. I do have my ID field set to auto increment where I see yours is manual

[ATTACH=JSON]{“data-align”:“none”,“data-size”:“medium”,“data-attachmentid”:90615}[/ATTACH]

Thank you. I did get it working … started over with a new form.
