not make create connection after first login

Not make create connection over first test prod environment.
Required create database connection and after completing the default password (scriptcase)
and completed change password always comes back and does not advance to Next step.

Production server in a intranet. Apache 2.2 + PHP 5.2.1 + ZEND 3.3.3
ALL TESTING server Is Work!.

The database used in Development is the same used in Prod ?
