populate fields after selection is made from combo box

Thanks to anyone who can help!
I have a very simple contact form. Name, Address, City, ST, ZIP etc. I have a zip code table populated with zip codes and corresponding city and state. Within my contact form the zip field is a combo box that grabs the data from the zip code table. I would like to have the zip codes as a combo box to select zip code. Once the zip code is selected I need to populate the city field and the state field.

I am very new to Scriptcase, so the simpler the example / explanation the better.
